To assess the Skin manifestations of organ recipients which are not uncommon findings in patients with end-stage renal disease (ESRD). As, Cutaneous examination of patients with ESRD has shown that 50-100% of patients have at least 1 to 5  dermatologic conditions in nail, skin, mucosa and hair. We assessed the dermatological manifestations among kidney transplant recipients due to chronic kidney disease. Special non invasive tool like dermoscopy was used. Report of twenty-six patients were assessed. The mean age of the patients was 47 years, with almost half of the patients (46%) from 41-60 years age group. Among the patients, 75 % were male, 65% were married, 80% were literate, 34% were past smoker and 44% were drinker. Skin manifestations were infections (Bacterial 13%, Viral-Herpes zoster 76% and Superficial fungal infections 88%), inflammatory conditions 18% like (lichen planus, Actinic keratosis, porokeratosis, psoriasis); xerosis and hyperpigmentation 91%, malignancy 0.5% (SCC, BCC, keratoacanthoma, melanoma) , Parasitic infection including scabies 15%. Non invasive tool was useful to reach in diagnostic for nail fungal infection, hair fungal infections, scabies burrows, melanoma and early herpes. A high prevalence of cutaneous disorders is expected, because most patients with organ transplant have an underlying disease process with cutaneous manifestations in developing country are increasing and the management, diagnosis of this disease is very expensive compared to other chronic diseases. Skin manifestations are due to drug therapy, dialysis, organ transplant and poor nutrition. Early diagnosis with help of dermoscopy and management is important to lower the cutaneous disorders as well. Dermoscopy training to all young dermatologist is essential for correct diagnosis.
