The risk of depression, suicidal plan, and suicidal attempt in chronic kidney disease: Results from the Korean national health and nutrition examination survey 2013-2018

Objectives: Chronic kidney disease (CKD) patients have a high prevalence of psychological disorders such as suicide and depression. CKD patients with psychological disorders were under-recognized, under-treated, have a low quality of life, and have a poor prognosis. The aim of this study was to evaluate psychological disorders in Korean CKD patients.  Methods: A total of 31798 participants who were participated in the Korean National Health and Nutrition Examination Survey from 2013 to 2018 were enrolled. We defined CKD patients as eGFR <60 or has proteinuria. The estimated glomerular filtration rate was calculated using the modification of diet in renal disease (MDRD) equation.  Results: Compared to non-CKD participants (n=27,481), CKD participants (n=4,317) were in depressed status (2.5% vs. 3.2%, p=0.012) and had more suicidal thoughts (2.3% vs. 2.8%, p=0.004), suicidal plannings (1.2% vs. 1.7%, p<0.001), and suicidal attempts (0.5% vs. 0.8%, p<0.001). Multivariate logistic regression analysis, compared to non-CKD participants, CKD participants was associated with depression [odds ratio (OR) 1.22; 95% confidence interval (CI) 1.00-1.49], suicide attempt [OR 1.56, 95% CI 1.04-2.33] and suicidal plan [OR 1.33, 95% CI 1.02-1.73]. In subgroup analysis, CKD participants with female [OR 1.71, 95% CI 1.24-2.35] or CKD participants below 65 years old [OR 1.78, 95% CI 1.28-2.46] were more associated with suicidal attempts or plans. CKD participants with female [OR 1.29, 95% CI 1.03-1.62] or CKD participants above 65 years old [OR 1.40, 95% CI 1.07-1.84] were more associated with depression.  Conclusions: Individualized evaluation and management strategies regarding depression and suicidal attempts should be taken into Korean CKD patients.
