Case Study: Urinary tract obstruction can be caused by prostate cancer or ureter stone and can be seen with hydroureter(s) and are accompanied by acute renal injury when obstruction is bilateral or patient has single functional kidney. Rarely fungus ball can cause obstruction and acute kidney injury. Here we are reporting a rare case that fungus ball in the ureter was diagnosed and successfully treated with intravenous antifungal agent as well as amphotericin irrigation via percutaneous nephrostomy tube and we hope to illustrate clinical manifestation and treatment of methods thereof. Although candiduria or candida species growing in urine culture is relatively common in hospitalized patients and usually benign, invasive candida infection is rare and sometimes difficult to distinguish from bacterial infection. However, the diagnosis should be suspected when infection persists despite adequate antibacterial therapies were given, because invasive fungal infection, when missed, can lead to complications such as perinephric abscess, fungus ball, prostate abscess or renal transplant infection. These complications often needs catheter drainage or surgical treatment as well as antifungal antibiotics treatment and are sometimes difficult to overcome. Candida species takes up the vast majority of fungal infections of the kidney and bladder. Urinary tract infection by Candida species occur by two mechanisms; ascending from the lower urinary tract and hematogenous dissemination to the kidneys.
