Feasibility assessment of catheter complications in peritoneal dialysis patients using a common data model

Objectives: A well-functioning catheter is a cornerstone for adequate peritoneal dialysis (PD). PD catheter is a medical device which was implanted, combined with other components (adaptor and transfer set), and used with drug (dialysate) for function. Therefore, monitoring of patient's health outcome of the PD catheter may be the simplest model of medical device vigilance (MDV). Common data model (CDM) is a logical and semantic data model that can be used to standardize multiple data sources into a common format. Methods: We conducted this study to check the current status and devise the measures to feasible utilizing CDM data for the medical device vigilance of PD catheters. We included patients with PD catheter insertion from January 2001 to 2021 July and analyzed events that seemed to be complications of PD catheters from the observational medical outcomes partnerships (OMOP)-CDM. Terminology for the procedure and medical device of PD by Health Insurance Review and Assessment Service (HIRA) was utilized as a vocabulary. Results: Total 351 catheters were implanted in 323 patients for PD. Total 175 catheters were removed in 161 patients. Total 1443.3±96.3 and 1378.0 ± 95.1 days were implanted days and PD days, respectively. Infectious complications including 108 peritonitis, 42 exit infection and 8 tunnel infection were reported in 135 catheters. There were various mechanical complications in 60 catheters. Intervention for complications was done as followings: 14 cuff shaving, 13 herniorrhaphy and 5 repositioning procedure. Although 315 (89.7%) catheters were searched in CDM, there is inconsistency of PD procedure. Although Infectious complications by CDM was detected in 130 catheters (96.2%), there is zero non-infectious complication. Conclusions: In this study, the result through CDM of PD complications was different with that by chart review. It is necessary to understand EMR, to define concepts and to standardize procedures and diagnoses in PD patients for MDV using CDM.
