- Protective role of silymarin in type 2 diabetic nephropathy and cardiomyopathy in combination with metformin in streptozotocin- high fat induced diabetic rats

Objectives: Accountability of Type 2 diabetes is more than 90% among the diabetics, associated microvascular and macrovascular complications are extremely distressing for patients and put a huge burden on the health-care systems. Insulin resistance and a progressive loss of pancreatic beta-cell activity linked to rising hyperglycemia are two features of type II diabetes and its complications. This study was undertaken to investigate the role of combined effect of silymarin and metformin in nephropathy and cardiomyopathy.
Methods: Low dose of streptozotocin (35 mg/kg) and a high fat diet (HFD) were used to develop diabetes. The levels of different renal and cardiac markers were estimated. The levels of different antioxidant parameters were also checked to confirm the antioxidant effect.
Results: Combined treatment with metformin (50 mg/kg) and silymarin (25 mg/kg, 50 mg/kg and 100 mg/kg) showed antidiabetic activity by significantly decreasing blood glucose, cholesterol, TG and LDL and increasing HDL levels. The combination is nephroprotective as a significantly decreased urinary albumin, sodium, creatinine and BUN. Cardio protective effects are proven by a significant decrease in collagen levels and also decrease in plasma levels of CK-MB, LDH, CRP in LV. It also brought significant decrease in LPO and NO, while increase in the superoxide dismutase (SOD), catalase (CAT) glutathione (GSH)and total thiol(T-SH) levels in both renal and cardiac systems as compared to the diabetic control group. Histopathology revealed decrease in glomerular lesions, mesangial expansion and tubular atrophy in renal tissue as compared to diabetic control rats. Moreover, histopathological examinations of LV cells showed less reduction in extracellular space, extent of fibrosis and cardiac fiber disarray was also reduced.
Conclusions: The results suggest that combination of silymarin and metformin returned the levels close to normal in the diabetic complications. These results suggest thatthe combination exhibitshypolipidemicas well as hypoglycemic effects in diabetes and its complications.
