- A case of acute tubulointerstitial nephritis with ingestion of hibiscus sabdariffa

Hibiscus sabdariffa is one of the most commercially available herbal tea in the world and has been reported to antihypertensive and hypocholesterolemic effects in animal models. However, there has been no report of acute tubulointerstitial nephritis (ATIN). A-22-year-old female was visited to the Gastroenterology department with a history of nausea and poor oral intake. Over the pas two months, she had been drinking Hibiscus sabdariffa tea every day. Initial laboratory finding exhibited blood urea nitrogen of 22.9 mg/dL and creatinine of 2.67 mg/dL, and urinalysis showed urine 1+ proteinuria, sterile pyuria with 5-9 white blood cells. Renal histology revealed tubular injuries and infiltration of inflammatory cells, such as plasma cells, neutrophils, and eosinophils consistent with ATIN. We administered prednisolone and renal function was restored to the normal level after 1months. We report the first case of ATIN that occurred in a woman who ingested Hibiscus sabdariffa tea for 2months.
