This lecture aims to: · Define patient reported outcomes (PROs) and examine its importance · Discuss PROs prioritised by patients receiving dialysis · Understand how PRO is shaping the direction of clinical care and research Definition of Patient reported outcome (PRO) PRO is a health outcome directly reported by the patient who experienced it. PROs prioritised by patients on dialysis Despite the intrusive nature of dialysis, PROs are not captured adequately in dialysis. The work led by the global Standardised Outcomes in Nephrology (SONG) initiative has identified several PROs of importance to key stakeholders. Core outcomes, defined as outcomes of critical importance to all key stakeholders include, life participation for PD and fatigue for HD. · Life Participation o Defined as the ability to participate in activities, including but not limited to paid or voluntary work, housework, study, travel, hobbies, family duties, social events, recreation, and leisure activities o No study has specifically reported life participation in comparing HD vs. PD o Variations of life participation have been reported favouring PD over HD (or comparable results) across: § Employment § Functional status § School attendance o Work is underway to develop a validated measure by the SONG-PD initiative · Fatigue o Fatigue is one of the most frequent complaints among patients receiving dialysis especially for haemodialysis o Outcome comparison between dialysis modalities are limited to subset within the domain of HRQOL o Currently an exercise intervention RCT is underway, evaluating fatigue as a primary outcome (M-FIT trial; co-designed with patient partners) in dialysis. Summary · Whilst we acknowledge the limitation of evidence from scarce high-quality outcomes – PD appear to confer advantages over HD across various PROMs · More work needs to be done to validate and broadly implement PRO measures (e.g., life participation)
