Objectives: Renal biopsy is an essential procedure for diagnosis and prognosis of the glomerular diseases, however needs a lot of skill and experinece and usually requires admission for a couple of days unlesss sometimes associated with life threatening complications like AV fistula, nephrectomy, uncontollable bleeding. We have an experience of more than 3,000 cases of renal biopsy without major complications, however all patients were admitted for a couple of days. Since 10 years ago we performed 1,848 cases of renal biopsies at OPD level without admission. Methods: Before renal biopsy we checked bleeding tendencies. Blood thinning agents were discontinued for more than 1 week. Anesthesia were done by local by lidocaine but sometimes used IV ketamine for young children who are uncooperative. Disposable kidney biopsy needle (TSK lab) were inserted under the ultrasound guide (LOGIQ 10S). We checked perirenal hematoma, AV fisutula etc every one hour for 4 to 6 hours until discharge. During last 10 years. we performed 1,849 cases of renal biopsies at OPD level without admission. Pathology reading was done by renal pathology specialist. Results: All renal biopsies were done at OPD level without admission. Three patients were developed hematomas measuring 5 cm but there were no AV fistula and uncontollable bleedings. Biopsy results were as follows: IgA nephropathy 621(33.6%), MesPGN 304 (16.4%), FSGS 236 (12.8%), Focal nonspecific glomerulonephritis 196 (10.6%), Diabetic kid disease 79 (4.3%), Tubulointerstitial nephritis 74 (4.0%), Podocyte disease 52 (2.8%), Allergic purpura nephritis 52 (2.8%), Minor change 52 (2.8%), Membranous nephropathy 43 (2.3%), Obesity related glomerulopathy 26 (1.4%), ESRD 22 (1.2%), Alport syndrome 16 (0. 86%), Membranoproliferative glomerulonephritis 13 ( 0.7%), Lupus nephritis 12 (0.64%), C1q nephropathy 9 (0.49%), Others 56 (0.3%). Conclusions: Although need a lot of skill and experiences, OPD level renal biopsy might be a safe procedure if done properly
