IgA nephropathy is a primary glomerular disease characterized by IgA deposition in the glomerular mesangial area, and is a syndrome with diverse and heterogeneous clinical and pathological manifestations. The pathogenesis of IgA nephropathy is complex, and the overall prognosis is poor, with limited conventional treatments and a lack of specific therapeutic approaches. Telitacicept, a dual-target receptor-antibody fusion protein, reduces proteinuria as well as delays the decline in glomerular filtration rate in patients at high risk for progressive IgA nephropathy. This article describes a patient with IgA nephropathy who presented with nephrotic syndrome after delivery and reached partial remission with the application of ARB and hormone therapy, but because of her markedly elevated BMI, reached severe obesity, she switched to treatment with telitacicept, which resulted in an improvement in her condition and a marked reduction in proteinuria, achieving clinical remission and improving clinical effi
