Factors Influencing the Incidence of Intradialytic Hypotension

Intradialytic hypotension is one of the complications that can occur during the hemodialysis process and associated with cardiovascular events and increases mortality. Intradialytic hypotension is related to ultrafiltration rate, cardiac output, and arteriolar tone. In 2022, Dialysis Unit Mayapada Hospital Jakarta Selatan had 42 patients died, 13 of whom had a history of intradialytic hypotension. In 2023, data was collected during the dialysis process, and 151 incidents of intradialytic hypotension were found and further investigation was carried out, 57% had a ultrafitration rate of more than 13 mL/kgBW/hour, 19% consumed anti-hypertension drugs before dialysis, 14% low ejection fraction, 10 % anemia. The interventions carried out include increasing the frequency of dialysis to prevent high UF rates, regularly coordinating with cardiology, not taking antihypertensive drugs in patients with a history of hypotension, and regular laboratory examinations. The result is that at the end of 2023, the incidence of intradialytic hypotension was only 7 incidents out of 174 patients. Patients with a history of intradialytic hypotension will undergo further monitoring and intervention.
