Epstein-Barr Virus (EBV)-Associated Acute Interstitial Nephritis in an Adult Diagnosed as Thyroid Cancer

Epstein-Barr Virus (EBV) is a rare cause of acute interstitial nephritis (AIN) in adults. Here, we describe a 31-year-old female with EBV-associated AIN. She was admitted due to elevated serum creatinine levels (2.9 mg/dL), proteinuria (2+), anemia, and leukopenia detected during a health examination. Additionally, ultrasonography suggested the presence of a thyroid mass and multiple neck lymphadenopathies. After admission, she complained of fever, sore throat, and fatigue. Her past medical history was not significant. Initial laboratory tests revealed a serum creatinine level of 3.4 mg/dL, leukopenia, and anemia. She was started on intravenous ampicillin. A kidney biopsy revealed extensive interstitial inflammatory infiltrates, with Epstein-Barr encoding region (EBER) in situ hybridization positive, consistent with EBV-associated AIN. EBV IgM was positive with an elevated viral load. She was started on treatment with prednisone at 1 mg/kg/day. Her symptoms and renal parameters improved after a 7–10-day course of prednisone. She continued to take tapering doses of prednisolone in the outpatient clinic. One month later, she was presented with oliguria, fever, and general weakness. A repeat kidney biopsy was consistent with EBV-associated AIN, and her viral load had increased again. Her renal function deteriorated, and pulmonary edema occurred. She was started on hemodialysis (HD), and the prednisolone dose was increased to 1 mg/kg/day. Renal parameters and her symptoms improved one week after starting HD. She underwent a fine-needle aspiration for the thyroid mass and was diagnosed with papillary thyroid cancer. After a total thyroidectomy, she was treated with mycophenolate mofetil and a low dose of steroids. Her recent creatinine level was 1.6 mg/dL, and she has no significant complaints.
