Ambulatory PDCI, complications of outpatient post-PDCI, peritoneal dialysis, peritonitis, peritoneal dialysis outcomes

BACKGROUND: The National Kidney and Transplant Institute (NKTI) is one of the many institutions in the Philippines that promoted the Peritoneal Dialysis (PD) First Policy. Inpatient PD-initiation among end stage kidney disease (ESKD) patients has been increasing in our institution which has contributed to more hospitalizations and exhaustion of medical resources. The ambulatory or outpatient PDCI, which was recently launched in 2022, has been of help in decreasing unnecessary hospital admissions. OBJECTIVES: To determine the outcomes of ambulatory PDCI in ESKD patients in the National Kidney and Transplant Institute (NKTI) A single-center, descriptive, retrospective study. Data was collected from the electronic medical records of all patients who underwent ambulatory PDCI from August 1, 2022, to December 31, 2023 A total of 28 patients were included in the study, with 98% of them followed-up for at least more than a year. PD-associated infection was more common than non-infectious complications. The PD peritonitis rate was 0.54 episodes per patient-year, followed by exit-site infection and tunnel site infection. PD catheter migration was the most common non-infectious complication. Death-censored technique survival was 71%. The mortality rate was 14%, with most deaths due to non-PD-related causes. PD-associated infections were more frequent than non-infectious complications among patients who underwent ambulatory PDCI. The overall outcomes were comparable to those of inpatient PDCI at our institution.
